چکیده

Objective To describe a novel RoboSling technique performed at the time of robot-assisted radical prostatectomy (RARP) and its utility for enhancing urinary function recovery postoperatively. Materials Methods The surgical involves harvesting vascularised, fascial flap from peritoneum on posterior aspect bladder. Following completion prostatectomy, autologous is tunnelled underneath bladder incorporated into rectourethralis vertical longitudinal detrusor fibres neck with modified Rocco suture. After urethra-vesical anastomosis completed, corners are hitched up to Cooper's ligament bilaterally V-Loc sutures, tensioned secured creating sling. A prospective, cohort study was 193 consecutive patients undergoing RARP between December 2016 September 2019. first 163 underwent standard RARP, last 30 had concurrently. Continence outcomes were primary assessed using pad number Expanded Prostate Cancer Composite (EPIC)-urinary domain questionnaire. Operative (OT), estimated blood loss (EBL), complications oncological secondary outcomes. Results two groups comparable demographics clinicopathological variables. At 3 months, zero usage (p = 0.005) continence rates, defined as EPIC score ≥ 85 0.007), both higher in group. EBL, complication rate positive margin did not differ groups. Superior observed 1 year group 0.029). added average 16 min OT. Conclusions procedure associated earlier return without negatively impacting other postoperative This improvement maintained long term.

Laparoscopic radical prostatectomy with bladder neck preservation: positive surgical margin and urinary continence status

INTRODUCTION Preservation of the bladder neck (BN) has been controversial, as limited excision of the bladder neck may result in incomplete resection of the disease. Moreover, the urinary continence rate may not be improved. AIM To evaluate the effect of bladder neck sparing on urinary continence, and surgical margins status in prostate cancer (PCa) patients treated with laparoscopic radical ...

Bladder neck preservation during radical retropubic prostatectomy and postoperative urinary continence.

INTRODUCTION Bladder neck-sparing modification of radical retropubic prostatectomy has been reported to lower the risk of urinary incontinence after prostatectomy. We reviewed the outcomes in men with prostate cancer who had undergone prostatectomy with either bladder neck preservation or bladder neck reconstruction. MATERIALS AND METHODS In this retrospective study, a total of 103 patients w...

Bladder neck preservation improves time to continence after radical prostatectomy: a systematic review and meta-analysis

Bladder neck preservation (BNP) during radical prostatectomy (RP) may improve postoperative urinary continence, although its overall effectiveness remains controversial. We systematically searched PubMed, Ovid Medline, Embase, CBM and the Cochrane Library to identify studies published before February 2016 that assessed associations between BNP and post-RP urinary continence. Thirteen trials (11...

Effect of bladder neck preservation during endoscopic extraperitoneal radical prostatectomy on urinary continence.

OBJECTIVE The current study investigates the effect of bladder neck (BN) preservation on postoperative continence and positive surgical margins (+SMs). PATIENTS AND METHODS 150 patients (group 1) who underwent BN-sparing endoscopic extraperitoneal radical prostatectomy (EERPE) and 90 patients treated with EERPE and BN resection (group 2) were retrospectively evaluated. RESULTS Both groups w...

Influence of bladder neck suspension stitches on early continence after radical prostatectomy: a prospective randomized study of 180 patients.

Several techniques have been introduced to improve early postoperative continence. In this study, we evaluated the impact of bladder neck (vesicourethral anastomosis) suspension on the outcome of extraperitoneal endoscopic radical prostatectomy (EERPE). In this research, a total of 180 patients underwent EERPE. Group 1 included patients who underwent nerve-sparing EERPE (nsEERPE) (n=45), and Gr...
